Creating form in SurveyMonkey
Step 1. From the SurveyMonkey home page, select the "Login" option to log in to your user account and start the process of creating questionnaires. If you have not registered with the platform, make a free fast registration;
Click on "Login"
Step 2. Once logged in to SurveyMonkey, click on "Create Questionnaires" in the top bar to open the form-drawing tool;
In your profile, click "Create Questionnaires"
Step 3. On the questionnaire creation page, there are three alternatives for the user. The first, "Most Popular" displays templates most commonly used by SurveyMonkey users, while "My Questionnaires" displays templates previously created and registered in your account;

Step 4. Already in the "All Models" option, dozens of models of the most diverse themes are displayed. You can select each one and edit the questions and answers to create a new questionnaire;
In "All Templates" it is possible to choose questionnaires already ready, but with editable questions and answers
Step 5. Press "Start Zero" to create a blank questionnaire, without using any of the templates offered by the platform;
In "Start from Zero" start a new model without any of the previous settings
Step 6. At the beginning of the process of creating the questionnaire, choose a name for it, define its thematic area and the button "Create Questionnaire";
Name and rate your questionnaire and then press the highlighted button
Step 7. There are four steps in the process of creating questionnaires. In the first one, in the tab "Elaborate Questionnaire" it is necessary to define the questions and to classify the type of answer that will be used. SurveyMonkey offers 14 categories of responses, 12 of them in the free version. Click the check box next to the question text field to set the response category;

Step 8. Then, depending on the response category you choose, write and define the order of the options that will be displayed. When you finish the question settings, select the "Save" button;
Finish the questionnaire by defining the answers that will be offered and press "Save"
Step 9. The question and answers will appear the way they will be presented in the questionnaire. To insert a new question, click the "New Question" button;
To continue constructing the questionnaire, press "New Question"
Step 10. Your questionnaire can also be divided into sections called "Page". To create a new section, go to the bottom of the page in the question elaboration interface and hit the "New Page" command;

Step 11. The new sections will display the title of the questionnaire, with options for inserting subtitles and changing nomenclatures;
In the initial part of the new page it is possible to change the subtitle and visualize the logical relations of the questions
Step 12. SurveyMonkey also allows you to establish a logical relationship between the questions - defining whether the answer to a question makes you skip others or follow the sequence of the questionnaire. To create a logical relationship, click on an already created question and click on the "Logic" tab. Determine the actions of the questions at the bottom, with the answers to the question;

Step 13. Click on the top tab "View and Evaluate" the process of creating the questionnaire to proceed to the next step. In this interface it is possible to verify the format and presentation of the questions and answers of the questionnaire;

Step 14. Click on "Collect Answers" to advance to the next step. This phase offers different options for sending and sharing the questionnaire created, among them: link creation, sharing by social networks or sending by e-mail;

Step 15. The last alternative of this interface, "Manual Entry of Data" allows inserting answers in the questionnaire itself to test it or of questionnaires applied non-electronically;

Step 16. "Manual Data Entry" also displays a history of the inserted registers, helping to control their insertion.
